I've seen this before. It happens when the BOW line is released before the bridle-sling is unhooked from the winch. Sternline 1st, then bridle-sling disconnected from the davit, THEN the bowline payed-out from the cleat it was secured on.
Just my 2cents of expirence -cw
Its like he said some one let the bow line go before getting unhooked I drove for years and would only let certain crew members hold my bow line, This can all be avoided if you just let the boat stop, There was a young guy killed on the Madrugador because of a david ram beaking and the whole david swung down and hit him body was never even recovered because the boat was moving so fast.
